Important Update: DentaQuest & Hawki Insurance

As of July 1, 2026, Iowa added DentaQuest as a secondadministrator for Iowa Medicaid's Dental Wellness Plan (DWP) and the Hawki Children's Health Insurance Program (CHIP). Delta Dental of Iowa continues to administer both plans as well — members are assigned to one administrator or the other, so you may see either "Delta Dental" or "DentaQuest" on your card for the same Hawki benefit.

Harlan Dental is in-network with Hawki (CHIP) only — under both Delta Dental and DentaQuest. We are not in-network with the Dental Wellness Plan (DWP) or adult Iowa Medicaid, regardless of which company administers it. If you have any questions, call the number on the back of your insurance card — or reach us at 712-755-3466.

In-Network Providers

We're In-Network With:

🦷

Delta Dental Premier

In-network coverage for all adult patients.

👶

Hawki (CHIP)

Coverage for children 18 and younger — through both Delta Dental and DentaQuest.

Please note: Our office does not accept Medicaid or the Dental Wellness Plan (DWP), whether it's administered by Delta Dental or DentaQuest — we are Hawki-only, not DWP/Medicaid. If you are uninsured or your plan isn't accepted, please consider our HD Membership Plan as an affordable alternative.

Out of Network

Out-of-Network? No Problem.

At Harlan Dental we are committed to providing high-quality care to all our patients, regardless of their insurance provider. While we may not be in-network with your plan, you can still receive care at our office through your out-of-network benefits.

1

Flexibility of Choice

You can always choose to receive care at our office. Many insurance plans provide out-of-network benefits that help cover a portion of your treatment cost.

2

Reimbursement Process

Reimbursement varies by insurance company and can be made directly to you or to our office. We will continue to file claims with your insurance on your behalf.

3

Potential Costs

As an out-of-network provider, your portion of care may differ depending on your plan. This could include a different deductible, co-pay, or the difference between our fee and what your insurer is willing to pay.

4

Maximizing Your Benefits

Our team is happy to help you understand your out-of-network benefits and provide an estimate of out-of-pocket costs before treatment begins.

A note about dental insurance: Dental insurance and medical insurance are not the same. Dental insurance is designed to be more limited, with a primary focus on preventive and basic dental care. At Harlan Dental, your oral health and satisfaction are our top priorities — we're dedicated to working with you to make care affordable and accessible, regardless of your plan.
